I hereby authorize the staff of Westonka Animal Hospital to perform anesthesia on my pet(s).

*I acknowledge that there inherently exists a risk of anesthetic complications during any anesthetic or surgical procedure. The complications can range from mild to life threatening.

* We do require preanesthetic blood screening for all pets with general anesthesia.A preanesthetic blood chemistry screen will reduce the risks of anesthesia by detecting problems with the liver and kidney. These two major internal organs (in addition to the lungs) are responsible for eliminating most of the anesthetics from the body. Although most patients have optimally functioning livers and kidneys, it is those few pets that do have an underlying, hidden problem and shouldn't have anesthesia at this time, that we want to screen for. Our goal at Westonka Animal Hospital is to make this surgical procedure as safe as possible for your pet.
The cost of the preanesthetic blood chemistry screen testing is $63.90.


*For the safety of all patients in the hospital ward, all surgery patients must be current on rabies and distemper vaccinations. Proof of previous vaccinations must be presented to the hospital. Patients vaccine records which cannot be confirmed by the time of surgery, will have them done at the owners expense.

[    ]  Prior to the release of your pet we ask that you have a discharge appointment. This appointment can be any time after 10 AM on the day that your pet can be released. There is no charge for this brief appointment during which a technician will go over home care for your pet and answer any questions. We ask that you make the appointment when your pet is admitted to the hospital.

Hours available for surgery discharge are:
Monday, Tuesday, Thursday: 10:00 a.m. to 5:45 p.m.
Wednesday: 12:00 p.m. noon to 5:45 p.m.
Friday: 10:00 a.m. to 5:45 p.m.
Saturday: 10:00 a.m. to 11:45 a.m. (we close at 12 noon on Saturdays)

A Calm and Restful Environment
Since Westonka Animal Hospital is a temporary home for sick, and sometimes scared, pets, it is our responsibility to provide the most calm and restful environment possible. Therefore, if your dog barks to the point of disturbing other patients while staying with us, we will use a citronella collar on your pet to control the barking. The citronella collar is designed to spray a small amount of "lemon-like" citronella when the dog barks. Humans tend to like the smell of citronella whereas dogs tend not to like it. The spray is directed out away from the dog; the sound and the smell are unpleasant enough to dogs that they will be distracted from barking. Most dogs learn very quickly and stop barking within a minute of having the collar on. It does not shock the dog or in any other way cause pain or harm, though you may notice a slight smell of citronella when you pick up your pet.
