The American Association of Feline Practitioners and the Academy of Feline Medicine recommend
that all cats six months of age or older have a blood test for Feline
Immunodeficiency Virus (FIV) which is similar to the human virus HIV. FIV
is not the same virus as Feline Leukemia Virus.
While your pet is under anesthesia, we would like to take the opportunity to take a blood sample so we run the sample in clinic and have the results in a matter of minutes. Two to three percent of all normal, healthy
appearing cats in the United States are FIV positive ! The
cost of the test is $53.83. Would you like to have your cat blood tested for FIV virus while he/she is under anesthesia?
Please circle your answer: YES NO
Would you like a free copy of the current recommendations for FIV testing to go home with? YES NO
To learn more about FIV, on the internet, vist the Feline Imminodeficiency Virus Information Center at http://web.vet.cornell.edu/public/fhc/fiv.htm
Westonka Animal Hospital makes great efforts to reduce postoperative complications but complications can and do occur. Therefore, I understand that postoperative complications may occur. I understand that the follow up examinations are at no charge to me, but that I am financially responsible for any charges relating to any medications, surgery, anesthesia, or supplies recommended by Westonka Animal Hospital to remedy the complication.
Note to Spay surgeries: Some conditions require additional surgery time and there will be an additional charge.
Note to Declaw surgeries: - before declawing, have you considered using a scratching post or nail trimming as alternatives to declawing? It is our recommendation that cat owners should consider these alternatives and others before declawing. Declawing should be considered to be a last resort to cat owners. Click here if you would like more iformation on alternatives to declawing. Laser surgery will decrease surgical pain experienced by your cat but will not eliminate it. Even if performed with a laser, the declaw procedure should be considered to be an elective, potentially painful surgical procedure.

[ ] Yes !! I authorize Westonka Animal Hospital to perform laser surgery on my pet.

Pet intestinal parasties are a potential health risk to their owners. For more information about intestinal parasites and their threat to humans, visit the Centers for Disease Control information at http://www.cdc.gov/ncidod/diseases/roundwrm/roundwrm.htm
Often times our clients have a difficult time getting their pet’s fresh stool to the hospital for an Intestinal Parasite Examination. Intestinal parasites, such as roundworms and hookworms are infectious to people and can cause blindness in children. This test should be done yearly in cats that go outdoors and once every two years for indoor cats. If your cat defecates while at the clinic, would you like for us to collect it and test for parasites?
[ ] Yes, please check my pet's stool. (pet & human health investment is $21.12)
[ ] No, I would not like my pet's stool checked for intestinal parasites at this time.
I have read and agree to the above statements: Signed:_____________________________
