Which Forms Do I Need?

New Clients
New visitors to our clinic should complete a New Client Information Sheet. Existing clients do not need to complete this form.

Anesthesia
All pets requiring general anesthesia will need to have the Anesthesia Consent Form. completed

Cat Neuters, Cat Spays, &/or Cat Declaws
You will need to have completed only the Anesthesia Consent Form and the Cat Spay/Neuter Consent Form.
Regarding the declaw procedure: we encourage cat owners to declaw their cat only as a last resort after they have tried other declaw alternatives.

Dog Neuters, Dog Spays
You will need to have completed only the Anesthesia Consent Form and the Dog Spay/Neuter Consent Form

Teeth Cleaning or Other Dental Procedures
You will need to have completed only the Anesthesia Consent Form and the Teeth Cleaning Visit Consent Form

Other Surgery
You will need to have completed only the Anesthesia Consent Form and the General Surgery Consent Form
